43 y/o man with a history of total anomalous pulmonary venous drainage s/p corrective surgery at age 3, found to have a systolic murmur over the LUSB, which is brought about by lifting the left arm above the level of the shoulder. This likely represents turbulent flow either from pulmonary venous stenosis at the anastomotic site, or possibly a subclavian bruit (perhaps he had a catheter in the L subclavian during and after surgery). The patient is asymptomatic and no additional testing has been performed at this time. However, he did have a recent TTE that showed mild stenosis of the anastomosis between the pulmonary veins and left atrium.
